These <string_error> issues that I am seeing more and more of, have they always been there, or is it this relatively new? People with DB/SQL experience, is this indicative of a larger problem?

They appeared with the Ice Games on Ghallanda for me.
I don't remember them being there before that...
On the other hand at that time I was playing on Codemasters Servers... and while we had a 1 to 4 month delay in modules ( supposedly for French and German Translation ), we had clean modules without crashes, with most of the bugs fixed and a lot of other stuff that never made it on US servers fixed.

Seems Turbine made a build, sent the source code of the server and client to CM, which then did its own QA and localization, and then compiled it.

They've been around since the beginning.

Usually some odd mob "voice" text or name, occasionally a spell/effect description in the combat log. A lot have come and gone.

These are only indicative of a larger problem insofar as they indicate someone put faulty data either into the code or the database and the expected record for the data isn't located.  They don't spread, they don't cause other issues, but they certainly look sloppy.

I agree they have been here since the beginning.  I wouldn't say those are indicative of a larger issues per say it could be as simple as typo but based on how bad MYDDO is I have always had a strong inclination that their database is a mess.  Normally, if you give someone an AccountID and say query 100 pieces of info from this account it should be a trivial task.

The only time that's a cascade issue is if they decided to refactor the DB search methods, or ordering reference schema. Wouldn't necessarily rule either of those out, since I believe you're referring to old stuff dissapearing, like some of the map descriptions in gianthold and the like and not the newer stuff,  but call it highly improbable.

I just checked linkedin and I have a contact that is a contact with a DB guy they just hired at Turbine about 4 months ago. If he is at the same level as my linkedin contact, he knows his sizzle.

basically all text is kept in string table files, this allows them to create multiple language files and simply have the code point to the correct one for each language. The problem arises when the code refers to an id that does not exist in the table.

It has been there since the beginning, but has gotten worse with more text being added. QA will let it pass through because it is non game breaking, and Turbine would rather have the release out in the wild and fix later than hold it back to get some more polish.

It's not a hard mistake to make, in my understanding.  Like, if they put $Shared_Bank in the code and it's actually $shared_bank, then you're gonna get some hop ons.

That makes sense, too.  I was just thinking about KoL, which I'm totally getting back into right now - they recently had a major issue along those exact lines, and their developers are like the anti-Turbine, so they shared every step of fixing it with the player base while accepting feedback and responding to it. 

It's kinda fucking magical.
