I am the head of the QA in my company (actually I am the only one in the QA).
I had some important information about some specific residues from desinfectants used in packing houses, that are not allowed in Europe. I sent an email to a very important provider in South America at the 8th of May. The recipients were the CEO, the QA director, the QA assistance and the Comercial Department. The Subject "MUY IMPORTANTE: blah blah..."
2 months later this provider had to take out from the european markets 3.000.000 kg of fruits : "We didnt know that".

People just dont give a fuck about what QA is saying or writing or asking for.
