Hand: Epic Sword of Shadow

Cloak: Not sure! Using drow cloak atm as filler
Ring1: Str +7 Seal of House Dun'Robar with Stunning +10
Ring2: Encrusted Ring (20% healing amp incredible potential)
Trinket: Epic Gem of Many Facets (GFL yellow)
Your neck slot is weak. I'd do either:
Epic Necklace of Venom for the SFL, Protect +5, Poison on Vorpal, -20% threat (you aren't a tank). This frees up your yellow slot on the Epic Gem. And makes better use of the gem to give you two set bonuses from it.
Epic Grims bracelent- +7 str, dodge 3%, green slot(love green slots). Switch your dun'robar ring to +2 con variant as your helm is giving you the +7 con and +2 str. (Or get rid of the helm and combine the +2 con or+2 str on the ring with a loot gen +8 con helm or wait for a black or white dragon helm with +8/+3 str/con stat on it)
As you mentioned, the Cloak is the other questionable slot. Two suggestions:
Epic Cloak of Night would be a good fit in the Cloak slot if you have one. Gives you the deathblock, 2% dodge, DR/5, and ghostly.
Admantine Cloak of the Bear isn't bad for you as it will give you some defensive stats and the +6 Exceptional Combat Mastery.
Otherwise, it looks pretty solid.
EDIT: Might want to consider Dragon Scale vs. Dragon Plate if you do the Epic Grims and/or Epic Cloak of Night. With the Dun'Robar you will be at 9% dodge on those items, which is wasted on the Heavy Armor.
