I think that the combat log cant keep up with the attacks done, and much of the issue is probably there.
Not sure anyone has looked at it recently, but you get your initial main attack, then there is a new line for every effect after it - basically a wall of text on every attack. Then there is a pause while the client tries to keep up. I think we are literally text flooding ourselves off the game (thus the jitteryness).
I manage to only have about three attacks in my maxed combat window.

Fellas! the combat log was only looked at in regards to no attacks being made, checking everything possible.
Animations show a throw, but that is it. no mob reaction, no mob damage, no mob aggro, no rolled dice, no special attacks, and no entry in the combat log, which was one of the last places checked to make sure I wasn't missing something anything. normally play with mine off.
And in the end it was TF throwing hammers that was the culprit in my case, the other weirdness?? got me!
