Piston Boots and Cornerstone Champion aren't critical. Fit in some sort of artifact. 3 filigree alone is huge. The Brand of Kalok Shash is great for this build if you can fit magical sheltering in a different slot.

I would definitely go for the Chrism though. Even if you have to shortman normal Baba 20 times for the runes. That, or LR to TWF.

Arcsteel Brim would be an option for MRR. Or maybe a slaver item.

Some things that say "while raged" actually function all the time. So it's worth it to drop rage for fort bypass against certain enemies if all you are losing is a few pts in Str. If that's too finicky for you, you could swap in a 3rd weapon focus (still not PA). Power Attack has been a poor choice since attack rolls started mattering.

Healing hands is arguably even more important in high skulls since you can't really count on anyone else being able to keep the healer up thanks to the big dumb hp bonus of EDF.
