You're probably running Essence of the Shrike which is WAI.
Re: Balanced Attacks, it doesn't always display the knockdown, but you should always see the dps spike after a vorpal. Some things are immune to knockdown though.
First of all, you need an item with melee alacrity like

http://ddowiki.com/page/Item:Jorgundal%27s_Collar_(Level_25). These are 12% and 15% which are, I believe, the best in the game.
Improved Critical:Bludgeoning is also a must to extend crit range.
Concord Op should be equipped - I actually keep it on until I'm L27/28, and only swap on EE.
Essence of the Shrike gives 20 temp sp on crit, which when you're attack speed is in the 35-45 range happens quite often, especially since your double strike should be in the 20-30 range also (can't check my toon right now)
Finally, with the attack speed and double strike, use any of the vampirism/lesser vampirism weapons (

http://ddowiki.com/page/Item:Ruby_of_the_Endless_Night) and you basically don't need to heal your self any more. Occasional Rejuv with empower heal and maxed out heal skill means you basically don't have to spend SP keeping your self alive. Most EH's I never shrine except to recharge clickies.
I'm running PDK 9D/9F/2R right now but I've also run 9D/9R/2F for a ranger past life and 9D/9Rogue/2Ranger Shadar for my rogue past life. Tried a ML 9D/9Barb/2C, but that was pretty bad
